Kehr’s sign describes referred shoulder pain from intra-abdominal injuries, classically a splenic rupture. It is caused by irritation of the diaphragm which is innervated by the phrenic nerve. Pain is then referred to the shoulder by shared cervical nerve roots C3-C5.
